Insurance Billing
I offer insurance billing for those
that have been in automobile accidents, or have been injured on the
job (workers comp).
How do I get my insurance to cover
my treatments? You must first have
a physicians prescription for massage stating the amount of treatments
needed. For example, a doctor may prescribe massage therapy 1x per week,
for 6 months.
After you have a doctors prescription,
you should call your insurance company and let them know that you have
been referred for massage therapy and you are going to begin treatments.
If it is an auto-related accident, call your car insurance company and
they will give you a claim number and a claim representative. If it
is workers comp, then call them and get your claim number and all
the information you will need from them (as stated below).
Then, call me and we can start the
treatments. Be sure to bring your original Doctors prescription,
your insurance claim number, and insurance companys info like; their
name, your claim representatives name and number, fax, phone number,
date of your accident, and their address.
Then, off you go to your massage therapy treatment! I will bill it out for you! No hassle for you.
If the insurance company fails to pay for some reason, you will be responsible for all charges for your massage services.
Let me know if you have any questions about this or your case.
If you have private insurance that covers all or a portion of your massage treatments, I can fill out a standard health insurance claim form for you to submit to you insurance company, as well as receipts for your sessions.
Policies
Appointments: I
work by appointment only. Please call me to set up your appointment.
All appointments must be paid for in full at the time of service unless
otherwise arranged or if covered by your insurance company.
Cancellation: I request at least 24 hours cancellation notice on all appointments. Your appointment time is reserved for you, so if you do not give proper notice, you will be charged for the session in full.
If you forget your appointment and do not call, you will be charged in full for the appointment. If your sessions are being billed to the insurance company, they will not be billed for a missed appointment. You are responsible for payment of any missed sessions.
I do not make appointment reminder calls unless you request them. You are responsible for your appointment times and the above fees for missed appointments or last minute cancellations.
Arrival: For your first appointment with me, please arrive a few minutes early (10-15 minutes). This time allows you to fill out the initial intake form, as well as to discuss your health concerns. Keep in mind that the session starts at the time scheduled and ends at the time scheduled. So if you are late arriving, unfortunately, I cannot add any extra time to the session.
Please turn off your cell phone for the session! (This is for your relaxation purposes!)
Payment: I accept cash, check and credit cards. Payment is due at time of service. An additional $30 fee will be charged for returned checks.
Confidentiality: All health information or personal conversation during a massage is strictly confidential. This information will only be released with the clients written consent or if legally subpoenaed.
Scope of Practice: I am not a doctor; therefore, I do not prescribe or diagnose any condition. I can and will refer you to a doctor if I feel it is needed.
My massage and bodywork is strictly therapeutic. I will not allow any sexual connotation or energy. If I feel that anything is inappropriate, I reserve the right to promptly end the session with full payment required.
Illness/Hygiene
Please know that if
you (or I) are sick, it is definitely not in our best interests to get
(or give) a massage. Use your best judgment in determining if you are
healthy enough to receive a massage and call me with as much advanced
warning as possible so we can reschedule.
Coming to your session clean and showered is greatly appreciated.
Not only will I be clean, so will my space. Clean sheets will be put
on the table prior to every client and I will only use scented oils
upon your approval. Please alert me to any allergies prior to arrival
so I can accommodate you.
I reserve the right to refuse treatment to anyone that I feel may pose a threat to my health or their health
